Missing include files.

Ok, I believe the postgres.h and all the files in the include/utils
directory need to be copied over during the installation, it got most of
the others, but missed those ones and as I've spent a good chunk trying to
get PHP and a few other utils built, they kinda needed them.

7.1 release candidate 2 has this issue.

This has been discussed quite a few times recently. From Tom Lane:

"Change php_pgsql.h to refer to postgres_fe.h not postgres.h."

You need to change PHP to pick up the NEW header.

PHP 4.0.5 should have this fixed.
