Documentation on disk usage

Since we just had the discussion about index size, I have written a
section for our administration manual that shows ways of finding the
disk usage for specific tables and databases:

I also updated README.oid2name to show a sample session that analyzes
disk usage.

It's much easier to use the routines in contrib/dbsize.

Done, though dbsize does not address indexes or TOAST tables. I have
added documentation to SGML to show how to find these values.
