CREATE CONVERSION/DROP CONVERSION implemented

I have committed support codes for DROP CONVERSION. I think most of
works for CREATE CONVERSION/DROP CONVERSION have been done.

Remaining works are:

o Add some conversion functions (mostly cyrillc encodings)
o Add SQL99 CONVERT command
